Patient Guide: Your Private Adult ADHD Assessment
This step-by-step guide explains everything you need to know – before, during and after your ADHD assessment.
Step 1: Booking your Assessment
- No GP referral needed.
- Click the Book Online link on our website.
- Fill in the Booking Form.
- Cost: £950 (pay in full when booking). Please have your debit or credit card ready.
- After payment, you’ll receive a confirmation email.
Who can use this service?
- You must live in England, Wales, Scotland or Northern Ireland.
- This service is not available for current UK armed forces members.
Choose your treatment
- Select Adult ADHD Assessment.
A pop-up message will be displayed, requesting your consent for Psychiatry UK to share your personal and medical data with your GP, including relevant medical history and current medications. This is a condition of service that is in your best interest to ensure safe medical practice and effective continuity of care.
UK law and policy recognise that appropriate information sharing is often integral to safe care. Notably, the Health and Social Care (Safety and Quality) Act 2015 introduced a statutory duty to share information for care. It mandates that relevant health professionals “must ensure” a patient’s information is shared with others providing care to that patient, where it will facilitate care and is in the patient’s best interest. Psychiatry UK reserves the right to discontinue treatment if essential medical information cannot be shared.

- You’ll receive a Welcome email.
- Set up your MedQare patient portal account.
- Complete the Medical History Questionnaire.
Step 2: Preparing for your Assessment
Checklist items
Portal Profile
- Go to ‘My Profile’ in your MedQare dashboard.
- Fill in all personal details.
Identity Verification
- Upload photo ID (passport, driving licence etc.).
- Upload proof of address (utility bill, bank statement, etc.).
- ID verification is mandatory for online assessments.
Pre-assessment Questionnaires
Found under Forms > Pending Forms in your portal:
- Adult ADHD Self-Report Scale (ASRS)
- ADHD Self Report
- ADHD Informant Report
- Heart Questionnaire
If you still have school reports, it’s helpful to upload these to your notes too.
- This must be completed by someone who knows you well (ideally since before age 12) and who has known you for the longest period of time (e.g. a parent, sibling, partner or friend).
- If you do not have anyone suitable, please let us know.
Why this matters: As ADHD is a developmental condition, we need objective collateral information from your childhood to assist your clinician with the assessment. Please do not complete this form yourself.
These help your consultant understand:
- Your daily life and routines.
- Your relationships.
- Your work or education experiences.
- Your family and medical history.
The assessment process involves gathering as much information and documentation as possible for your clinician to match against the ‘core’ characteristics of ADHD outlined in the DSM-5 (Diagnostic and Statistical Manual of Mental Disorders, 5th edition).
Standardised pre-assessment questionnaires are a structured way to start collecting this information and are instrumental in helping your clinician reach an accurate diagnosis.
We recommend putting aside some quiet time to gather your evidence and complete your forms, giving as much information as possible to help your consultant start building a picture of your life, your symptoms and the challenges they present.
Step 3: Booking your Appointment
You will only be able to book your appointment after:
- Uploading photo ID and proof of address.
- Submitting all four questionnaires.

- We’ll email and SMS a booking link.

- Read consultant profiles before choosing.
- Appointments are 7 days a week, 6am–11pm.
What you will need:
- Computer, laptop or tablet with camera + microphone.
- Test Your Device on our website.
- We recommend the Microsoft Teams app (no account needed).
- Otherwise, use the latest Microsoft Edge browser.
If you need to rearrange or cancel your appointment, please create a note on your portal for our medical secretary team as soon as possible.
If your appointment is due to take place in less than 48 hours, it will not be possible to cancel your appointment and it will be considered an Appointment Non-Attendance.
If you cancel with seven or more days’ notice, you will receive a full refund. If you cancel between two and seven days before, a 50% fee will apply. If you cancel within 48 hours, the full fee is payable.

Step 4: Accessibility and Adjustments
It is important that you feel comfortable during your assessment. If it’s helpful, you can request:
- Communication support (shorter questions, extra time, written prompts).
- Adjustments for sensory needs.
For deaf and hard-of-hearing patients:
We can provide:
- 90-minute appointments (instead of 60 minutes).
- Qualified interpreter (option to meet 10–15 minutes before session).
- Second interpreter if needed.
- Choice of sign language (e.g. BSL, Signed English).
- Live transcription available.
- Ask for clarification to reduce misunderstandings or errors.

Step 5: Your Online Consultation
- Duration: 50 mins-1 hr
- It will take place via Microsoft Teams.
- Find a quiet, private space.
- A family member can join if you would like.
At the start:
Your Consultant will introduce themselves.
- They’ll check your photo ID.
- They’ll ask if you’re happy for your assessment to be recorded (you can say no).
- Written notes are always taken for your patient record.
During the assessment:
- Please keep your camera on. Observation is an important part of the process.
- Your Consultant will follow a structured format to cover all diagnostic criteria.
They will:
- Review the pre-assessment info gathered.
- Ask about your history, experiences and current challenges.
- Gather a complete psychiatric history.
- Consider related conditions.
- Observation + discussion helps identify core ADHD characteristics.
Important:
- If a question feels uncomfortable, please tell your consultant. They will not pressure you to answer.
- If you think of additional info after the session, let your Consultant know that you will add a note in your portal after the appointment. Ask them to consider it for your diagnosis.
Step 6: Your Results and Next Steps
After your assessment:
Your Consultant will explain their findings.
- A diagnosis will be given, if appropriate.
- You’ll receive a written report within 7 working days.
- A copy of your report will be sent to your GP.
Your report will include:
- Your diagnosis (if applicable)
- Your personalised treatment plan
- Resources and support options
Your treatment plan will include:
- Medication treatment options.
- Non-medication options (therapy, coaching, skills training).
- Where to find ongoing support.
Treatment and Support Options
- If medication is recommended, you can ask to be referred to our Titration Service.

Your consultant may also recommend non-pharmacological treatment(s) which can be beneficial in helping to reduce the challenges of your condition.
There are a number of therapies that can help:
- Relationship or family therapy
- Coaching / life skills training
- Occupational therapy
- Employment advice /workplace adjustments
- Sensory support.
- Psychotherapy for related conditions
- Self-help books, websites, and online forums can help too.
